About CDC 1815

In 2018, the Tennessee Department of Health (TDH) was awarded a 5-year Centers for Disease Control and Prevention (CDC) 1815 Cooperative Agreement entitled “Improving the Health of Americans Through Prevention and Management of Diabetes, Heart Disease, and Stroke.”      

 To support 1815 goals and objectives, TDH utilizes funding to conduct activities in the following areas:

  • Increase access to, coverage for, and enrollment and retention of people with prediabetes in the National Diabetes Prevention Program (National DPP) lifestyle change program. 
  • Work with healthcare organizations to identify people with prediabetes and refer them to CDC-recognized organizations offering the National DPP lifestyle change program.
  • Increase access to, coverage for, and participation of people with diabetes in Diabetes Self-management Education and Support (DSMES) programs recognized by the American Diabetes Association (ADA) or accredited by the Association of Diabetes Care & Education Specialists (ADCES). 
  • Increase the number of pharmacists who provide DSMES and help people with diabetes manage their medications. 
  • Increase reporting and tracking of clinical data to improve identification, management, and treatment of patients with high blood pressure and cholesterol. 
  • Encourage providers to follow evidence-based guidelines and policies for team-based care of patients with high blood pressure and cholesterol. 
  • Increase community-clinical links that support systematic referrals, self-management, and lifestyle change for patients with high blood pressure and high blood cholesterol. 
  • Encourage patients to continue taking their medicine for high blood pressure and cholesterol. 
  • Assist patients with high blood pressure and cholesterol manage their conditions. 
  • Encourage more patients with high blood pressure and high blood cholesterol to join proven lifestyle change programs.
