The StEM Coalition
The goal of the TN Diabetes State Engagement Meeting (StEM) is to expand access and participation in the National Diabetes Prevention Program (NDPP) and Diabetes Self-Management Education and Support (DSMES) programs ensuring that all Tennesseans have access to high quality diabetes prevention, management, and supportive care.
The coalition brings together 150+ key partners in diabetes prevention and management who have been working toward reducing diabetes prevalence. The coalition consist of organizations from various sectors including, but not limited to:
- Health System Partners
- Community- and Faith-based Organizations
- Local NDPP Providers
- Academic Partners
- Public Health Partners
- Non-profit Organizations
TN StEM consists of three (3) regional work groups, following the three (3) grand divisions in TN, including West, Middle, and East. Regional work groups determine a sustainable strategic plan that supports resident’s health, particularly among high-risk populations, by helping them prevent and manage diabetes.
- Middle focuses on opportunities for promoting DPP and increasing employer/insurer coverage.
- East continues efforts to reach medical providers and organizations to promote DPP to increase referrals.
- West focuses on DPP promotion at local diabetes awareness events and activities.
