*1/14/16 UPDATE TO THIS SECTION: The extended public comment period has ended. TennCare is no longer accepting comments on the revised Statewide Transition Plan, but that plan is still accessible below for review.

*12/15/2015 UPDATE TO THIS SECTION: TennCare is extending the comment period for the revised Statewide Transition Plan an additional 30 days. The previous deadline for comments was December 13, 2015. TennCare will now accept comments through January 13, 2016. You can find the Plan and information about commenting under the “Transition Plan” subheading on this webpage.

*12/11/2015 UPDATE TO THIS SECTION: TennCare has posted a memorandum explaining the State's heightened scrutiny review process for home and community-based settings related to the federal home and community-based services settings rule. That document is available under the HCBS Settings Resources tab on this page.

*11/13/2015 UPDATE TO THIS SECTION: TennCare has posted a revised version of the Statewide Transition Plan posted on November 2, 2015. Changes made to the November 2, 2015 are indicated with highlighting in the track changes document. TennCare has extended the comment period on the revised Statewide Transition Plan to December 13, 2015 as a result of this revised posting.

*11/2/2015 UPDATE TO THIS SECTION: TennCare has posted a revised Statewide Transition Plan, which amends the previous Statewide Transition Plan posted on 1/26/15. The Statewide Transition Plan posted today has been updated based on results of State, provider, and contracted entity self-assessment of compliance with the HCBS Settings Rule. All changes from the 1/26/15 version are in track changes. Please see the Document “Revised Proposed Statewide Transition Plan with Track Changes - November 2, 2015” below. TennCare will post the link to submit comments in the coming days. The comment period will continue for 30 days once the link is made available.

*2/24/15 UPDATE TO THIS SECTION: An updated crosswalk for the Residential Provider Self-Assessment has been included directly following the Residential Provider Self-Assessment Tool in the list of documents below.

*2/6/15 UPDATE TO THIS SECTION: Based on feedback from HCBS providers, an FAQ document regarding provider self-assessments has been created to assist providers in completing the self-assessment and is available in the list of documents below.

*2/3/15 UPDATE TO THIS SECTION: The Individual Experience Assessment (IEA) Tool has been revised based on feedback received after the IEA trainings were completed, and the tool is available in the list of documents below. IEAs are to be completed by January 31, 2016 with each person receiving residential and/or day services. These assessments will be conducted by Independent Support Coordinators, DIDD Case Managers, and Managed Care Organization Care Coordinators as appropriate.

*1/26/15 UPDATE TO THIS SECTION: The clean version of the revised Proposed Statewide Transition Plan that was sent to CMS, which incorporates public comments, is included below. Additionally, a track changed version of the same document is also included below to indicate language that was amended based on public comment.

*1/23/15 UPDATE TO THIS SECTION: The public comment period for the Proposed Statewide Transition Plan has ended. Additional guidance from CMS concerning Non-Residential Settings has been included immediately after the documents listed below.

*12/23/14 UPDATE TO THIS SECTION: The Proposed Statewide Transition Plan is included below with track changes to identify changes from the waiver-specific transition plan, which was approved by CMS. TennCare is accepting public comment on the Proposed Statewide Transition Plan beginning today through January 22nd, 2015. Comments can be provided by accessing the first link after the list of documents below.

*12/19/14 UPDATE TO THIS SECTION: The Individual Experience Assessment Tool is being reviewed by the Department of Intellectual and Developmental Disabilities and TennCare’s contracted Managed Care Organizations. The revised tool will be posted by the end of January 2015.

*12/5/14 UPDATE TO THIS SECTION: A revised Employment Data Sheet is included below. The Individual Experience Assessment Tool has been temporarily removed from the site, and a revised version of that tool will be posted the week of December 8th, 2014.

*11/13/14 UPDATE TO THIS SECTION: The final version of the Transition Plan was approved by CMS on November 7, 2014. The Approved Transition Plan is included below. Additionally, the Provider presentation on the self-assessment process has been modified based on provider feedback and is included below.

*11/7/14 UPDATE TO THIS SECTION: The final version of the Transition Plan, which has been submitted to CMS for approval, and a revised version of the Residential Provider Self-Assessment Tool, have been posted below. Additionally, we are adding the Department of Labor-Home and Community-Based Services Provider Training Invitation for the Provider Trainings that were conducted in July 2014 regarding the new Department of Labor and CMS HCBS Settings and Person-Centered Planning Rules.

*11/3/14 UPDATE TO THIS SECTION: The Non-Residential and Residential Provider Self-Assessment Tools have been revised and the finalized tools are available below. Providers will also receive word versions of these documents from either a Managed Care Organization or the Department of Intellectual and Developmental Disabilities, as applicable.

*10/28/14 UPDATE TO THIS SECTION: Non-Residential and Residential Provider Self-Assessment Tools are now available for providers to use to determine compliance with the HCBS Settings Rule. A copy of the Provider presentation regarding how providers can complete and submit the Non-Residential and Residential Provider Self-Assessments and the validation process for these self-assessments is also available.

*10/24/14 UPDATE TO THIS SECTION: A notice relating to mandatory webinar sessions for HCBS Residential and Day Program service providers is now available. The notice contains a list of required participants, as well as dates, times, and additional information for the webinars, which will train Residential and Day Program service providers on how they will complete and submit their Provider Self-Assessments and/or Transition Plans.
